Medications
All medications must be kept in the clinic with the proper forms completed. No medication is to be carried by a student or kept in a student's belongings or locker.
*Non-Prescription Medication Form (PDF) - brought in original container accompanied by attached form.
*Prescription Medication Form (PDF) - brought in original container accompanied by attached form that is signed by parent and physician.
*Self-Carried Medication Form (PDF) - This form must be accompanied with the prescription medication form. Student's physician must authorize self-carried / administered medication. Student name must appear on the (inhaler, container).

Physicals
The Ohio Department of Health requires a physical to be obtained before entrance to pre K and Transitional Kindergarten and every 13 months thereafter.
Immunizations
Students are required to provide a record from a physician's office of all required immunizations by the 15th day of school or the student may be excluded from attending. If the parent declines immunizations for reasons of medical, religious, or good conscious an exemption form needs to be completed.
Emergency Medical Authorization
This form must be completed at the beginning of each school year for each student:

Vision and Hearing Screening
This screening will be provided for all Kindergarten, 1st, 3rd, 5th, 7th, 9th graders and any students new to the school. You will be notified if your student fails either of the screenings. This is only a screening and should not replace a regular eye exam. If your student needs an eye exam and/or glasses and you are not able to afford them, please see your school nurse to give you information on how you may obtain free ones or ones at very low cost.

When Should My Child Stay Home?
Exclusion from school should occur if student display any of the following symptoms until symptom-free for 24 hours without medication:
- Oral temperature ≥100°
- Vomiting more than once and presence of other symptoms
- Diarrhea more than once
- Difficult or rapid breathing
- Severe coughing
- Rash with undetermined origin (must have physician permission to return to school)
- Unusually dark urine
- Grey or white stool
- Yellowish skin or eyes
- Eye discharge, eye pain, eye redness
If the person has seen his/her physician and has been diagnosed with a communicable disease, he/she needs to be on antibiotic, antiviral, or antifungal, or anti-parasitic medication for 24 hours or otherwise noted by physician before returning to school.
How can I be certain about confidentiality?
The privacy of students and their families regarding their personal health information is important. Nurses must maintain the nurse/patient relationship in compliance with the nurse practice act while determining nursing assessment and safe intervention of the circumstance. Clinic Aides, school secretaries that assist clinic needs, volunteers and any clinic staff must safeguard the privacy of students and their families. Information should only be shared that is necessary and in some cases the parent may need to be contacted as to the possibility of sharing health information.
Does the clinic need my volunteer hours?
YES!, MD's, and RN's are needed at various times throughout the school year, also at screening times in the fall. Please call your clinic to schedule with us.
Your donations are also appreciated as we often need to restock our shelves of frequently used items such as:
- 7-UP or Sprite-small bottles preferred
- Saltine crackers- Premium and Zesta are both nut free and the preferrable brand.
- Peppermints or Spearmints
- Ziplock plastic bags of all sizes
