DEFINITION: Administration of medication in a school setting.

OBJECTIVE: Dayton Christian Schools, Inc. desires to provide a safe confidential environment for all of our students, faculty and staff.

PROCEDURES: MUST HAVE WRITTEN OR FAXED PERMISSION FROM PARENT OR GUARDIAN FOR ALL MEDICATIONS. A WRITTEN PHYSICIAN REQUEST (if this is a prescription) IS ALSO REQUIRED. We DO NOT accept telephone permission. Permission forms are available in the school office or clinic, or you may print one from this website.

*Please note, the medications and forms for medications are not renewable. Each school year, new forms and medications are to be filled out and sent in.

It is strongly recommended that only those medications that are absolutely necessary be given during school hours. If possible, the medication or treatment should be given before or after school. It is requested that the first dose of any medication be given at home. Adults and children vary in response to medications.

It is required that all medications are in the ORIGINAL container (check expiration dates) with proper permission forms. When possible, parents should bring the medication to the office; otherwise, the student is to take the medication to the school office or clinic at the beginning of the school day.

No medication will be given to a student that does not follow the guidelines (proper permission, students name, original container, instructions on label matching instructions on permission form, stop and start dates).

NO NARCOTICS will be given at school, i.e. Codeine, Demerol, Darvon, cough syrup containing narcotics, etc. If in doubt, contact your School Nurse.

*Dayton Christian Schools, Inc. does not have, hand out stock, nor medication for students.

NON-PRESCRIPTION MEDICATION

Parent permission in writing, name of medication, original container with same medication, students name, dosage (matching that of label), times to be given, reason to be given (state "for any pain" or specific use), start and stop dates.

PRESCRIPTION MEDICATION

Physician permission (Emergency room Physician, Urgent Care Physician), Parent permission, name of medication, dosage, times to be given, method of administration, original container, possible side effects, contact phone numbers. If prescription changes are made, proper paperwork is needed.

NO students are to carry non-prescription or prescription medications during school hours. All medication is to be kept in a locked location in the clinic or office, unless doctor permission (Epipens, glucometer, glucagons, insulin pump, inhaler, etc. must be age appropriate/responsible to carry. Proper forms must be kept in a medication book in the clinic. In some cases, a photo I.D. may be requested for students with severe allergies.

COUGH DROPS

Jr. High and High School need a note from a parent to carry cough drops. Elementary students need a note from a parent to receive cough drops and the cough drops will be kept in the clinic, office or designated medication location for your building.

*Students that carry medications or have medications in their lockers without permission will receive school discipline.

FIELD TRIPS

If prescription medication is needed during a field trip, the medication is to be sent from home in original container with only the one dose needed. Medication sent in baggies, etc. will not be given. The medication cannot be obtained from the clinic. The medication is to be given to the classroom teacher, and the teacher is the only person to give medication to the student, unless the students parent or persons on the Emergency Medical Form are on the field trip, then they can give the medication. Proper and current paperwork must be on file in the clinic.

AFTER SCHOOL CARE

No non prescription medication will be dispensed by after care workers. All prescription medication will be handled on a case by case basis.

*Non-Prescription medications not picked up at the end of the school year will be discarded or donated to a Teacher's area.

*All prescription medication not picked up at the end of the school year will remain in the office and after two weeks will be discarded or placed aside for faculty and staff use.

These regulations are in accordance with Section 3313.713 of Ohio Law regarding administration of medications to students at school.
