New Student Enrollment - Step 4

• If you checked the box indicating your need for student transportation during the 2012-2013 school year, and your school district does not provide busing, please complete the Dayton Christian School Transportation Registration form.

• If you checked the box indicating a need for Extended Care (Grades Pre-K - 5th), please complete the Extended Care Request form.
